50 an hour 2 days ago 2 days ago 2 days ago The Charles County Department of Health is seeking a highly motivated and detail-oriented Family Obesity/Chronic Disease Program Coordinator for our White Plains, MD office.
The main purpose of this position is to develop family healthy weight programming to increase physical activity and proper nutrition among Charles County children and families.
The incumbent will address health disparities in obesity and co-morbidities by implementing evidence-based programs such as Mind, Exercise, Nutrition Do it! (MEND), the National Diabetes Prevention Program (DPP), Stanford's Chronic Disease Self-Management Program (CDSMP), Building Better Caregivers, and Healthy Heart Ambassadors.
The position also requires that the incumbent conduct community outreach on health education topics.

KEY FUNCTIONS/
Responsibilities:
Establishes a family healthy weight program and implements the program in the community to target and disparate populations.
Plans and implements identified services or strategies to address health disparities and equities in Charles County.
Develops public education programs and materials.
Conducts community outreach and education on health education and health equity topics including but not limited to physical activity, nutrition, chronic disease, etc.
Assists chronic disease and tobacco prevention and management staff with evidence-based programming such as Diabetes Prevention Program (DPP), Chronic Disease Self-Management Program (CDSMP), Building Better Caregivers, Healthy Heart Ambassadors, etc.
Participates in other community health projects as assigned by the Director of Community Health and Evaluation and the Health Officer.
GENERAL REQUIREMENTS Education:
Graduation from an accredited high school or possession of a high school equivalency certificate.
Experience:
One year of professional experience in assisting in the coordination of a health-related program(s) or referring people to governmental and private resources.
Notes:

Candidates may substitute 30 college credit hours from an accredited college or university for the required experience.

Candidates may substitute U.
S.
Armed Forces military service experience as a commissioned officer in Health Care Administration classifications or Civil Affairs Specialist specialty codes in the health-related field of work on a year-for-year basis for the required experience and education.
KNOWLEDGE, SKILLS, AND ABILITIES:
Knowledge of the principles, methods, and objectives of public health Knowledge of principles and methods of physical activity and nutrition guidelines Knowledge of obesity prevention strategies Experience in facilitation of large/small groups Ability to write clear and concise reports Ability to establish and maintain effective communication and working relationships with supervisors, agency leadership, co-workers, community partners and others Ability to communicate effectively both orally and in writing Job Types:
